Ask to see the form at any point and it opens on screen, so you can check what has been filled before anything is submitted.

This is the slowest way to use it — one question at a time — and the clearest way to see who does what. Answering several questions at once is considerably quicker.

Finished forms are rendered on your own practice letterhead, with practice name, address, CQC and GDC registration numbers and your logo, and submitted as a PDF carrying a unique content hash so an inspector can verify the copy they are given.
